WISE Adult Day Health

Grandmother and daughterWise Adult Day Health provides Skilled Nursing Care which includes the management of catheter care, continuous oxygen, ostomy care, wound care, diabetes management, g-tube feeding, medication management. Medication Management includes the dispensing of medications, injections, hand-held nebulizer treatments, eye drops, etc. Diabetic Monitoring includes the monitoring of blood sugars, diet management, education, administration of insulin as needed.

Preventative Healthcare- includes the monitoring of complex medical conditions to prevent complications, diet, exercise, proper nutrition, safety & supervision.  Personal Care Assistance is provided by trained C.N.A.’s who assist the individual with their Activities of daily living which includes assistance with Toileting, Ambulation, handwashing, feeding, incontinence care.  Wise staff are trained in Alzheimer’s Dementia Care which includes the management of sundowning and wandering. Our well-lit facility is handicapped accessible, barrier free with no stairs and is secured to deter elopement.

Our Occupational therapist approved therapeutic Activities programming includes daily activities bingo, board games, arts and crafts, word games, puzzles, Daily Exercise, walking group, iPad group, red hats groups, singalongs, Entertainment and Community Outings.

Our program provides nutritious meals and snacks daily in accordance with federal nutrition guidelines. We can manage physician prescribed diets that require special consistencies (i.e. thickened liquids, ground, puree, low calorie, aphasia, no salt added).

Transportation is provided by our agency, handicapped accessible vans and can accommodate wheelchairs.  Restorative therapies such as Physical, Occupational or Speech therapy can be arranged on site for individuals requiring these services.

Wise Adult Day Health is able to manage medical diagnosis of Alzheimer’s/Dementia, Diabetes, COPD, Stroke, CVA, Elderly, Disabled, Handicapped, Intellectual and Developmental disabilities, Mental health, aphasia, Parkinson’s disease, Autism, Traumatic Brain Injury, Neurological disorders, Frail elders, and other complex medical issues.
